Introdução: O Câncer de Pulmão (CP) é um problema de saúde pública e descrever o perfil
epidemiológico e molecular de uma região é essencial para ampliar o acesso a terapias mais
seguras e eficazes. Objetivo: Identificar e analisar o perfil epidemiológico, histopatológico e
molecular de pacientes portadores de CP, em Fortaleza-Ceará, entre os anos 2000 e 2017, com
complementação de dados sociodemográficos em 146 prontuários. Os marcadores ALK,
PDL-1, IFN-¿, MLH1, MSH2, MSH6 e PMS2 foram avaliados por imuno-histoquímica e o
TIL foi avaliado no hematoxilina-eosina. Métodos: Análise retrospectiva de 793 casos de CP
oriundos de hospital e laboratório de referência, na Região Nordeste do Brasil. Resultados:
Em 793 laudos histopatológicos, 46% eram mulheres e a idade média dos participantes foi 61
anos. O tipo histológico predominante foi o adenocarcinoma (32,3%), seguido pelo carcinoma
espinocelular (11,6%). Houve associação do sexo feminino com a faixa de 50 a 79 anos (p=
0,033) e com adenocarcinoma primário e metástases pulmonares (p<0,0001). O estágio I do
câncer de pulmão foi o mais frequente em todos os anos do período analisado (p<0,0001).
Houve relação entre a necrose e o estadiamento, no que se refere ao tamanho, principalmente
com T2a e T3 (p<0,0001). As mulheres sobrevivem mais que os homens (p=0,001). Nos
prontuários analisados (n=146), 69,9% dos pacientes estavam no grupo tabagistas/extabagistas,
com associação entre os mesmos e o grupo agropecuária, florestais e pesca
(p=0,002). Na análise dos marcadores (n=127), ALK+ ocorreu em 4,0% das mesmas e o
escore de proporção tumoral ¿ 50% para PD-L1 ocorreu em 5,2%. O ALK+ foi associado
com a ocorrência de metástases (p=0,003) e PLD-1+ com invasão perineural (p=0,0017). O
H-score mais encontrado no IFN-¿ foi o 2. A perda da co-expressão mais comum foi
MSH2/PMS2 (55,7%), seguida da MLH1/MSH2 (36,5%) e MLH1/PMS2 (n= 35,5%.).
Conclusão: O conhecimento do perfil epidemiológico, histopatológico e da expressão dos
marcadores moleculares em uma região é essencial para determinação dos pacientes
candidatos a terapia alvo e imunoterapia.

Introduction: Lung Cancer (LC) is a public health problem and describing the
epidemiological and molecular profile of a region is essential to expand the access to safer
and more efficient therapies. Objective: We seek to identify and analyze the epidemiological,
histopathological and molecular profile of patients with LC in Fortaleza-Ceara between the
years 2000 to 2017. Method: A Retrospective analysis of 793 cases of LC from a hospital
and reference laboratory in Northeast Brazil WITH complementing data from 146 medical
records. ALK, PDL-1, IFN-¿, MLH1, MSH2, MSH6 e PMS2 markers were assessed for
immunohistochemistry and the TILS was evaluated using H&E. Results: In 793
histopathological reports, 46% were women, average age 61 years, with 32.3% being
adenocarcinoma and 11.6% squamous cell carcinoma. There was an association between the
age range of 50 to 79 with females (p=0.033) and between primary adenocarcinoma and lung
metastases with the same sex (p<0.0001). 73% of patients with stage I, the same occurred
more frequently in most ages (p<0.0001). There was a relationship between necrosis and the
staging of the disease with regard to size, mainly with T2a and T3 (p<0.0001). Women
survive longer than men (p=0.001). In the analyzed medical records (n=146), 69.9% of the
patients were in the group of smokers / ex-smokers with an association between them and the
agricultural group, forestry and fishing communities (p=0.002). In the analysis of the markers
(n=127), ALK + occurred in 4.0% of them and the tumor proportion score ¿ 50% for PD-L1
was found in 5.2%.ALK + was associated with metastasis (p = 0.003) and PLD-1 + with
perineural invasion (p=0.008). The H-score most found on IFN-¿ was 2. The most common
loss of co-expression was MLH2/PMS2 (55.7%), followed by MLH1/MLH2 (36.5%) and
MLH1/PMS2 (n=35.5%). Conclusions: Knowledge of the epidemiological, histopathological
profile and the expression of molecular markers in a region is essential for determining
patients who are candidates for target therapy and immunotherapy.
