A presente tese objetivou compreender os aspectos facilitadores da prática e a vivência da ortotanásia para os profissionais de saúde e pacientes oncológicos em processo de terminalidade. Foi estruturada em três artigos/pesquisas, complementares em seus objetivos. O primeiro artigo objetivou investigar a produção científica sobre a prática de ortotanásia e cuidados paliativos em pacientes com câncer em processo de terminalidade. Realizou-se uma revisão sistemática da literatura por meio de busca por artigos no portal de periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (Capes), publicados em português, no intervalo de 2011 a 2016, que apresentavam os descritores: ¿Ortotanásia¿ AND ¿Câncer¿ (f = 6); OR ¿Cuidados Paliativos¿ AND ¿Câncer¿ (f = 70). Após seleção por meio de critérios de exclusão, analisaram-se 19 artigos. Os resultados evidenciam a existência da obstinação terapêutica. Por outro lado, reforçam a importância do processo de humanização da morte e da tomada de decisão sobre a ortotanásia feita por profissionais, pacientes e familiares. O segundo artigo objetivou analisar a percepção sobre o câncer e a ortotanásia para os profissionais de saúde de cuidado curativo e paliativo em oncologia. Realizou-se uma pesquisa qualitativa que contou com a participação de oito profissionais de saúde das equipes de cuidados curativos e paliativos do Instituto do Câncer do Ceará (ICC). Foram utilizados três instrumentos: entrevista semiestruturada, Teste de Associação Livre de Palavras (TALP) e Teste de Projeção sobre Fotos (TPF). As entrevistas foram analisadas por meio do software Iramuteq. Para a análise do TALP, as palavras de cada estímulo foram submetidas à técnica de redução de palavras, categorizadas e compreendidas por meio de análise simples de frequência. Para análise do TPF, foram contabilizadas as frequências, seguidas de suas análises de significados. Os resultados mostram que o câncer é percebido como sinônimo de morte, dor e sofrimento; e ortotanásia como cuidado e afeto. Evidencia-se a necessidade da aceitação da morte e da mudança de abordagem de cuidado de pacientes em processo de finitude, e que por meio dos cuidados paliativos é possível oferecer qualidade de morte e alívio da dor. O terceiro artigo objetivou compreender a vivência de pacientes oncológicos em cuidados paliativos exclusivos sobre o processo do morrer. Realizou-se uma pesquisa qualitativa, que contou com a participação de 10 pacientes com câncer, em processo de terminalidade, em cuidados paliativos exclusivos: cinco em atendimento domiciliar e cinco em internação hospitalar. Utilizaram-se três instrumentos, em duas etapas: na Etapa 1 foram feitas visitas sistemáticas, escuta livre e registros em Diário de Campo; e na Etapa 2 utilizou-se o Teste de Associação Livre de Ideias (TALI) e o Desenho-Estória com Tema (sobre o ¿eu hoje¿). Os discursos de escuta livre nas visitas foram compreendidos por meio de análise descritiva. Para a análise do TALI, as ideias emergidas foram submetidas à técnica de redução por meio de categorização das ideias evocadas e categorizadas. Os desenhos foram compreendidos a partir de suas análises de significados. Os resultados obtidos demonstraram que os pacientes, ao longo do tratamento, adquirem mecanismos de enfrentamentos das perdas diárias e da doença. Por vezes, essas defesas são usadas para negar a realidade e rejeitar a doença. Em conjunto, os resultados obtidos nesta tese apontam que os profissionais reconhecem a importância e os desafios da ortotanásia, e profissionais e pacientes apresentam dificuldades em lidar com a morte. Conclui-se a necessidade de ampliação de esclarecimentos em torno da prática de ortotanásia para profissionais de saúde e sociedade em geral.

The aim of this thesis was to understand the facilitating aspects of the practice and the experience of orthotanasia for health professionals and cancer patients in the termination process. It was structured in three articles / research, complementary in its objectives. The first article aimed to investigate the scientific production on the practice of orthotanasia and palliative care in cancer patients in the termination process. A systematic review of the literature was carried out through a search for articles in the journal portal of the da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (Capes), published in Portuguese, between 2011 and 2016, which had the following descriptors: "Orthotanasia" AND "Cancer" (f = 6); OR "Palliative care" AND "Cancer" (f = 70). After selection by means of exclusion criteria, 19 articles were analyzed. The results show the existence of therapeutic obstinacy. On the other hand, they reinforce the importance of the process of humanization of death and decision-making on orthotanasia by professionals, patients and family members. The second article aimed to analyze the perception about cancer and orthotanasia for health professionals of curative and palliative care in oncology. A qualitative research was carried out with the participation of eight health professionals from the curative and palliative care teams of the Ceará Cancer Institute (ICC). Three instruments were used: semi-structured interview, Free Word Association Test (TALP) and Photo Projection Test (TPF). The interviews were analyzed using Iramuteq software. For the analysis of the TALP, the words of each stimulus were submitted to the word reduction technique, categorized and understood through simple frequency analysis. For the TPF analysis, the frequencies were counted, followed by their analysis of meanings. The results show that cancer is perceived as synonymous with death, pain and suffering; and orthotanasia as care and affection. There is evidence of the need to accept death and change the care approach of patients in the process of finitude, and that through palliative care it is possible to offer quality of death and pain relief. The third article aimed to understand the experience of cancer patients in exclusive palliative care about the dying process. A qualitative research was carried out, with the participation of 10 patients with cancer, in termination process, in exclusive palliative care: five in home care and five in hospital admission. Three instruments were used, in two stages: in Step 1 systematic visits were made, free listening and records in Diário de Campo; and in Step 2, the Free Idea Association Test (TALI) and the Theme-Story Design (on the "I Today") were used. The free listening discourses on visits were understood through descriptive analysis. For the analysis of the TALI, the emerged ideas were submitted to the technique of reduction by means of categorization of the ideas evoked and categorized. The drawings were understood from their analysis of meanings. The results demonstrated that the patients, during the treatment, acquire mechanisms of coping with the daily losses and the disease. Sometimes these defenses are used to deny reality and reject disease. Together, the results obtained in this thesis point out that professionals recognize the importance and challenges of orthotanasia, and professionals and patients have difficulties in dealing with death. It is concluded that there is a need to expand clarifications about the practice of orthotanasia for health professionals and society in general.
